Medical Accommodations

At I-House, we are committed to fostering an inclusive and supportive living environment for all residents. We recognize that some students may require specific housing accommodations. Our goal is to ensure your housing supports your well-bring and academic success.

How to Apply for a Medical Accommodation

To request a reasonable accommodation, you must first submit a housing application. Once your application is submitted, the Medical Accommodation Request Form will be available through the main menu of the applicant/resident portal. Requests are reviewed on a case-by-case basis.

If you are requesting housing accommodations due to a disability, we require supporting documentation from your medical provider. 

Documentation must include:

Confirmation that you have a disability (you are not required to disclose your specific diagnosis)

  • A description of the accommodation or modification you are requesting
  • An explanation of how the requested accommodation is necessary in relation to your disability and your ability to live at I-House

Documentation must meet the following criteria:

  • The letter must be dated within the last 90 days.
  • This letter must be in a PDF format on the medical professional's letterhead with an ink or verifiable signature.
  • If you are currently in the United States and have been for more than 30 days, the letter must be from a medical provider based in the United States.

Accommodation requests involving room changes are subject to availability at the time of approval. We encourage you to submit your request as early as possible. Please allow a minimum of 7 to 10 business days for processing. This information will be kept confidential and will be used only for ADA accommodations.

Please Note: Filling out this form does not register you for the Disabled Students Program, which provides disability accommodations in academic programs. Once admitted to the University, students requesting academic accommodations are strongly encouraged to register as early as possible with the Disabled Students’ Program. ADA Housing Accommodation Verification Needed

Dear Medical or Mental Health Professional:

This student or resident is asking International House at UC Berkeley for accommodations related to the Americans with Disabilities Act (ADA) while living in university housing. By providing a full and complete response, you will help to support the processing of this student’s accommodation request, and you will reduce the need for us to return to you for additional information.

Please include the following in your letter, if relevant:

  1. The student/resident’s full legal name and date of birth.
  2. Describe your specialty, how long the student has been in your care, and the date of the student's most recent appointment.
  3. Verify that the student has a qualifying disability that impacts one or more major life activities. You do not need to state specific diagnosis.
  4. Explain the specific housing accommodation(s) required.
  5. Describe the relationship between the individual's disability and how the requested accommodation(s) or modification(s) are necessary for the individual.
  6. Your ink or verifiable digital signature, license information, and the date.  

Please provide the letter directly to the student so they can include it with their accommodation request.
